Down Payment Refund Policy for Southern Professional Alliance dba Solution 4 Obesity
Cancellations must be received by Solution 4 Obesity in writing, email or via fax.
Any cancellation thirty (30) or more days before the scheduled procedure will forfeit thirty percent (30%) of the initial down payment plus any additional expenses incurred by Solution 4 Obesity for the purchase of airline tickets for traveling companions to the client.
Cancellations made less than 30 days prior to the scheduled procedure will result in forfeiture of fifty percent (50%) of the initial down payment plus any additional expenses incurred by Solution 4 Obesity for airline tickets for traveling companions to the client. 
The client may reserve the right to use his down payment as a credit towards a future procedure providing the client agrees to deduct from the down payment any incurred expenses by Solution 4 Obesity for unused client airline travel.
Any cancellation that is the result of a death or serious illness will be handled on case by case basis
